These resources are aimed to support the prevention of pressure ulcers/injuries (PU/PI) in the long-term care phase following spinal cord injury (SCI).
Effective prevention at home requires comprehensive discharge planning, continued implementation of preventive measures, and access to appropriate equipment, home modifications, and support. Ongoing follow-up and access to specialised SCI teams are essential to maintain skin integrity and address emerging needs.

The materials in this section focus on supporting daily self-management, ensuring continuity of care and guiding when to seek specialised support if complications arise.

These resources are part of the Pressure Injury Recurrence Toolkit – an evidence-based toolkit developed to support healthcare professionals, patients, and informal caregivers in preventing recurrence of pressure injuries in individuals with spinal cord injury. The project is developed in collaboration with Wounds Australia.
