Home Care – Wound Care: Clinical decision making

About

These resources for health care providers engaged in wound care in the home care setting currently include two resources:

1. A work book for healthcare professionals aiming to support them in making structured, evidence-based clinical decisions when managing wounds in the home care setting. They focus on systematic wound assessment using recognised frameworks such as HEIDI, TIME(RS) and MOIST, and link assessment findings to appropriate treatment choices. Through practical case studies, the materials guide clinicians in identifying wound aetiology, recognising infection or ischaemia, selecting interventions, and determining when escalation or referral is required. The aim is to strengthen safe, consistent and person-centred decision-making in home-based wound care.

 

2. A clinical case story illustrating the importance of a hollistic approach to wound care in the home care setting, including the involvement of family carers.
